Beginning in Spring 2018, the Opioid Drugs Sign Off Form is required for all student-athletes participating in spring sports.
Please read the Opioid Drugs Fact Sheet and then print, sign and return the Opioid Drugs Sign Off Form to the health office before the beginning of the spring sports season.
